Valia Lifestyle is an out-of-network provider of Health Maintenance Organization (HMO), Preferred Provider Organization (PPO), and Point of Service (POS) insurance plans. You will need to pay for your treatment and can submit an out-of-network benefits request, known as a superbill, to your insurance company. 

Depending on your insurance coverage and individual plan, what you paid towards your treatment plan may count towards your deductible. Also, services such as laboratory studies and medications are likely covered under your health insurance plan. 

Understanding your healthcare expenses is crucial. With Valia Lifestyle, you receive clear and detailed Superbills, providing a comprehensive breakdown of the services you've received. This transparency empowers you with a complete understanding of your healthcare costs.

A superbill is an itemized list of all services we provide to you. A superbill contains vital information, like diagnosis and procedure codes needed by your insurance company for the services when you receive out-of-network care. 

Some insurance companies may refer to superbills as Charge Slips, Encounter Forms, or Fee Tickets.

If you don’t currently have insurance coverage, you can opt to pay as you go, paying for your treatment when you need it. We request payment at the start of each appointment.
