Valia Lifestyle is currently in-network with several commercial insurance plans, such as Aetna and Cigna. Please check with your insurance provider for details on your specific plan and coverage.

If we are an out-of-network provider with your health plan, payment is requested at the time of your treatment and we can help you submit an out-of-network benefits request, known as a superbill, to your insurance company.

Depending on your insurance coverage and individual plan, what you paid towards your treatment plan may count towards your deductible. Also, services such as laboratory studies and medications are likely covered under your health insurance plan.

Please note that our Ketamine Assisted Lifestyle Medicine (KALM) program, which offers Ketamine treatment, is not covered by insurance.

A superbill is an itemized list of all services we provide to you. A superbill contains vital information, like diagnosis and procedure codes needed by your insurance company for the services when you receive out-of-network care. 

Some insurance companies may refer to superbills as Charge Slips, Encounter Forms, or Fee Tickets.

If you don’t currently have insurance coverage, you can opt to pay as you go, paying for your treatment when you need it. We request payment at the start of each appointment.
